1. Parking permits are issued only after receipt by St Peter UCC of proof of insurance from the automobile insurance company insuring the vehicle listed on the application.
2. Parking permits are issuable only to students who will be currently enrolled in Lake Zurich High School for the 2024-2025 school year and must be displayed hanging from the rearview mirror in the front windshield of any permitted vehicle. Vehicles must be parked forward into the assigned parking space.
3. Only one permit will be issued for each vehicle. Permits are not transferable to other students or vehicles.
4. The award of a parking permit by St. Peter UCC is a matter of discretion on the part of St. Peter UCC and creates no property right in the person to whom the permit is issued.
5. Students who have been issued parking permits by St. Peter UCC for the current school year shall be permitted to park vehicles displaying valid permits at the St. Peter UCC parking lot between the hours of 6:00 AM and 5:30 PM, Monday through Friday during the school year. Exceptions to this rule must be requested and approved.
6. If a permitted vehicle is sold or becomes inoperable, the student must notify St. Peter UCC at rooms@stpeterlz.org to inform St. Peter staff that the permit hang tag will be displayed in a different car and provide a copy of the valid insurance card for the replacement vehicle. 
7. Each parking permit will pertain to an assigned parking space. Each vehicle may only be parked in the designated space unless blocked by snow or another vehicle, in which case an unnumbered space in the nearest unnumbered row of the parking lot shall be used (The THREE WEST ROWS are numbered. Do not use the row nearest the church). If a vehicle is left in the parking lot during snowfall periods, it may become plowed in. St. Peter UCC assumes no responsibility for plow damage to a parked vehicle, and assumes no responsibility for unplowing any vehicle for any reason.
8. St. Peter UCC parking lot parking permits remain the property of St. Peter UCC and must be surrendered upon request. 
9. St. Peter UCC reserves the right to recall or revoke permits for any reason or no reason at all, to deny permits, to suspend permits, to close the parking lot and deny entrance to permit holders and to change this policy.
10. Vehicles which are parked in the parking lot which do not bear a valid sticker in the proper location or which are parked in the wrong location will be towed at the car owner’s expense. Warnings may not be issued. Information regarding how to retrieve the vehicle can be found on signs posted in the parking lot. 
11. Permit holders operating vehicles in the lot shall possess a valid Illinois driver’s license and the required insurance for the vehicle.
12. Permit holders entering and exiting the St. Peter UCC parking lot shall obey all speed limits and traffic laws, and shall not produce excessive noise (loud sound systems, squealing wheels, etc.)
13. Permit holders shall not deposit paper, garbage, food, beverages or debris in the parking lot or on any St. Peter UCC property.
14. Permit holders shall not loiter in the St. Peter UCC parking lot.
15. My parent or guardian and I individually and jointly agree to execute the St. Peter UCC Parking Permit Liability Waiver related to St. Peter UCC parking program.
16. Permit holder’s parking rights may be revoked without refund for any rule violations made by passengers in their vehicle or by persons dropping them off in the parking lot. Violations shown on security cameras or reported by witnesses will result in a permit being revoked.
17. Driving more that 15 mph in the parking lot will not be tolerated.
18. Students who abuse the rules will receive one verbal notice. Continued actions will result in the parking sticker being revoked.
19. A local towing company of St. Peter UCC’s choosing will have the right to tow cars. The parents of the students will be held responsible for all fees incurred.

IMPORTANT INFORMATION

St. Peter United Church of Christ in Lake Zurich, Illinois (hereafter, St. Peter UCC) is committed to operating its property in the safest manner possible and holds the safety of its constituents in the highest possible regard. Participants in the parking program and parents authorizing their child(ren) to apply for parking permits must recognize, however, that there can be an inherent risk of injury (including death), damages, losses or liabilities involving drivers, passengers, or other persons who are on or are using St. Peter UCC property or facilities. St. Peter UCC insists that all users of St. Peter UCC property follow safety rules and instructions which have been designed to protect the user’s safety.

WAIVER AND RELEASE OF ALL CLAIMS

Please read this form carefully and be aware in registering yourself and/or your child or ward for participation in the St. Peter UCC parking permit program that: (1) you individually and on behalf of your minor child /ward will be waiving and releasing all claims for injuries you or your minor child/ward might sustain, and (2) will be assuming the obligation to defend and indemnify St. Peter UCC with regard to any injuries your passengers or the passengers of your minor child/ward may sustain, which arise out of, or in connection with, your and your minor child’s/ward’s participation in the St. Peter UCC parking program.

As a participant, or as a parent/guardian of a participant in the program, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume all liability and risk of any injury (including death), damage, loss or liability which I and/or my minor child/ward may sustain as a result of participating in any and all activities connected with or associated with such parking program.

I further recognize and acknowledge that myself and/or my minor child/ward, as the operator of a vehicle, have the ability to control who is allowed to become a passenger in that vehicle, and/or after becoming a passenger in that vehicle, myself and/or my minor child/ward have the ability to control whether that passenger will be driven onto St. Peter UCC property and thereby exposed to any risks or dangers that may arise on that property. Accordingly, when I and/or my minor child/ward allow any other person to become a passenger in my vehicle, or, when I and/or my minor child/ward drive a vehicle with a passenger in it, to or from St. Peter UCC property, I, individually and on behalf of my minor child/ward, agree to assume all liability and risk of any injury (including death), damage, loss or liability which a passenger may suffer, arising out of, or in connection with, my and/or my minor child’s/ward’s participation in the St. Peter UCC parking program.

I, individually and on behalf of my minor child/ward, do hereby fully release and discharge St. Peter UCC and its officers, agents, servants, and employees from any and all claims from injuries, (including death), damage, loss, or liability which I and/or my minor child/ward, may have or which may accrue to me or my minor child/ward, and which arises out of, are connected with, or in any way associated with the activities of the parking program.

I, individually and on behalf of my minor child/ward, further agree to defend, indemnify and hold harmless St. Peter UCC and its officers, agents, servants and employees from any and all claims resulting from injuries, (including death), damages, losses or liabilities sustained by my and/or my minor child’s/ward’s, passengers arising out of, connected with, or in any way associated with the activities of the parking program.

In the event of any emergency, I, individually and on behalf of my minor child/ward, authorize St. Peter UCC officials to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for myself or for my minor child’s/ward’s, immediate care and agree that I will be responsible for the payment of any and all medical services rendered.

These numbers may be used in the event that towing is being contemplated.
