IMPORTANT INFORMATION
St. Peter United Church of Christ in Lake Zurich, Illinois (hereafter, St. Peter UCC) is committed to operating its property in the safest manner possible and holds the safety of its constituents in the highest possible regard. Participants in the parking program and parents authorizing their child(ren) to apply for parking permits must recognize, however, that there can be an inherent risk of injury (including death), damages, losses or liabilities involving drivers, passengers, or other persons who are on or are using St. Peter UCC property or facilities. St. Peter UCC insists that all users of St. Peter UCC property follow safety rules and instructions which have been designed to protect the user’s safety.
WAIVER AND RELEASE OF ALL CLAIMS
Please read this form carefully and be aware in registering yourself and/or your child or ward for participation in the St. Peter UCC parking permit program that: (1) you individually and on behalf of your minor child /ward will be waiving and releasing all claims for injuries you or your minor child/ward might sustain, and (2) will be assuming the obligation to defend and indemnify St. Peter UCC with regard to any injuries your passengers or the passengers of your minor child/ward may sustain, which arise out of, or in connection with, your and your minor child’s/ward’s participation in the St. Peter UCC parking program.
As a participant, or as a parent/guardian of a participant in the program, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume all liability and risk of any injury (including death), damage, loss or liability which I and/or my minor child/ward may sustain as a result of participating in any and all activities connected with or associated with such parking program.
I further recognize and acknowledge that myself and/or my minor child/ward, as the operator of a vehicle, have the ability to control who is allowed to become a passenger in that vehicle, and/or after becoming a passenger in that vehicle, myself and/or my minor child/ward have the ability to control whether that passenger will be driven onto St. Peter UCC property and thereby exposed to any risks or dangers that may arise on that property. Accordingly, when I and/or my minor child/ward allow any other person to become a passenger in my vehicle, or, when I and/or my minor child/ward drive a vehicle with a passenger in it, to or from St. Peter UCC property, I, individually and on behalf of my minor child/ward, agree to assume all liability and risk of any injury (including death), damage, loss or liability which a passenger may suffer, arising out of, or in connection with, my and/or my minor child’s/ward’s participation in the St. Peter UCC parking program.
I, individually and on behalf of my minor child/ward, do hereby fully release and discharge St. Peter UCC and its officers, agents, servants, and employees from any and all claims from injuries, (including death), damage, loss, or liability which I and/or my minor child/ward, may have or which may accrue to me or my minor child/ward, and which arises out of, are connected with, or in any way associated with the activities of the parking program.
I, individually and on behalf of my minor child/ward, further agree to defend, indemnify and hold harmless St. Peter UCC and its officers, agents, servants and employees from any and all claims resulting from injuries, (including death), damages, losses or liabilities sustained by my and/or my minor child’s/ward’s, passengers arising out of, connected with, or in any way associated with the activities of the parking program.
In the event of any emergency, I, individually and on behalf of my minor child/ward, authorize St. Peter UCC officials to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for myself or for my minor child’s/ward’s, immediate care and agree that I will be responsible for the payment of any and all medical services rendered.
